    A Functional Capacity Assessment helps to determine your current abilities and level of independence. Through interview and observation,  the therapist assesses your ability to manage personal care, domestic tasks, and community access/participation and then makes recommendations for therapy or interventions.

  • Time required: 15 - 20 hours

    Following a Functional Capacity Assessment and Sensory Assessment the therapist will recommend therapy goals and intervention strategies. This service includes:

    • Functional & sensory assessments

    • Travel time

    • Goal development

  • Time required: approx. 6 - 8 hours

    Your therapist will assess your requirement for a Sensory Profile Assessment.  This assessment involves an evaluation of an individual’s sensory processing patterns.

    • Summary of Sensory Patterns

    *Please note that this assessment does not include goal planning or a full overview of the individual’s functional ability.

  • Time required: 10 - 12 hours

    The Autism Diagnostic Observation Schedule, Second Edition (ADOS-2) is an observational assessment of Autism Spectrum Disorders. It uses a semi-structured, standardised evaluation of communication, social interaction, play, and restricted and repetitive behaviours.

    Therapists observe clients as they work through modules tailored for the client. After evaluating the source information the therapist prepares a diagnostic report for the family.

  • Time required: minimum 20 hours (additional hours may apply for complex equipment)

    Assistive Technology describes any device or system which helps an individual to perform a task as independently as possible. Your therapist will recommend equipment that supports improvement in your ability to carry out everyday activities.

     Examples include:

    • Manual or powered wheelchairs

    • Electric scooters

    • Walking aids

    • Electric beds

    • Recliner chairs

    • Commodes

    • Hoists

    • Rails

    • Cooling vests

    • Pressure care equipment

    • Communication devices, etc.

  • Time required: 30 - 40 hours

    A Home Modification Assessment reviews your ability to access and move within your home environment. This assessment considers your safety and independence.  

    The OT may make recommendations for minor, complex, or temporary solutions. Our OTs work with funding bodies, builders, and project managers for home modification requirements. 

    • Initial assessment

    • Liaison & visits with builders

    • Completion of reports

    • Travel time to meet with trades people

     Examples include:

    • Ramps

    • Bathroom modifications

    • Kitchen modifications

    • Ceiling hoists

    • Platform Lifts

  • Time required: 20 - 30 hours

    SDA refers to accommodation for participants who require specialist housing solutions to assist with the delivery of their supports. This accommodation can also cater for extreme functional impairment or high support needs.

    This is a detailed service:

    • Comprehensive functional assessment (usually up to 3 visits)

    • Interview with you and your care support team

    • Review of your circumstances against the NDIS SDA Eligibility Criteria

    • Comprehensive report

    • Travel time

  • Time required: approx. 20 hours

    Supported Independent Living (SIL) refers to any funded service supports which enable a greater level of independent living for participants.  SIL may include:

    • High-level support (more than 6 hours per day)

    • Highly trained staff

    Supports can be shared between 2 - 5 people.

    This is a detailed service entailing:

    • A comprehensive assessment - Interview(s) with your current care supports

    • Comprehensive report inclusive of recommendations towards your eligibility

    *Please note:  SIL can be provided in both SDA and non-SDA residences. 

  • Time required: approx. 20 hours

    Individual Living Options (ILO) are tailored living arrangements that focus on the participant and their individual housing support needs. These are often an alternative to a group home environment and may include care arrangements with non-professional carers. This is a detailed service:

    • Interview with you and your support team

    • A functional assessment

    • A review of your environmental needs

    • A comprehensive report

  • Time required: 8 - 10 hours

    This service provides details about:

    • The client’s disability and its impact on independence and goal attainment

    • The future need for NDIS funded services

    Participants find this service useful where:

    • They are transferring to the NDIS for the first time and require support and evidence for their planning session

    • They are on an existing NDIS plan and their individual needs or goals change

    • An NDIS plan review is pending

    The assessment covers:

    • Interview and observational assessment

    • Review of your past needs

    • Recommendations around future needs
